The Wolves chief revealed other players were also struggling.
"Kevin Foley has an ankle problem and he's had an injection. Stephen Hunt has had to have one as well butthose two will be fine."
Should Fletcher be out, Wolves would be down to two fit senior strikers in Kevin Doyle and Sam Vokes, at a time when they have not scored in two league games.
McCarthy revealed he tried to replace the injured Sylvan Ebanks-Blake before the transfer window shut, with Manchester United's Federico Macheda on his radar."
McCarthy, who watched QPR draw 0-0 at home to Newcastle last night, remains philosophical about the possible loss of Fletcher.
"The more positive you are, the more it rubs off on everyone else," he said.
"If they're injured, we'll give someone else a go."
