Lessons to be learned after deaths of six children

The deaths of six children have been reviewed by county health bosses, it has emerged.

The review was ordered earlier this year.

The deaths all took place between December last year and March this year, with five reported from Shrewsbury & Telford Hospitals NHS Trust (SaTH), and one from primary care.

All were said to involve "deterioration and sepsis", with a focus on the acute children's pathway through SaTH, which manages both Royal Shrewsbury Hospital and Princess Royal Hospital in Telford.

NHS Shropshire Telford & Wrekin said that a review had been launched in February – with the findings identifying a 'key theme' of "more timely communication regarding up-to-date clinical information being shared between organisations" .

Alison Bussey, Chief Nursing Officer at NHS Shropshire, Telford & Wrekin, said serious incident investigations had come up with some "immediate actions" as a result of the incidents.

She said they had included "focussed training for staff" and "development of newly registered nurses’ rotation programme across children’s services in both the hospital and community settings".
