West Midlands Ambulance Service spokesman Jamie Arrowsmith said: "Crews arrived to find a car and a van that had been in collision. The driver of the car, a woman, had suffered extremely serious injuries in the incident. Unfortunately, it quickly became apparent that nothing could be done to save her and she was confirmed dead at the scene.

"From the same car, a young girl was treated for serious head and abdominal injuries.

"She was anaesthetised in order to stabilise her condition before being flown to Birmingham Children's Hospital. Unfortunately, her injuries are thought to be extremely serious.

"A second girl, who was the front seat passenger, was treated for a leg injury and suspected chest injuries before being taken to Hereford County Hospital."

The condition of the young girl who was airlifted is being described as "critical".

The Midlands Air Ambulance, two land ambulances, paramedics and a doctor were called to the scene at about 3.30pm along with fire crews and police.

The road was closed between the junction with the A417 at Hope under Dinmore and the A4103 Roman Road north of Hereford.
