Telford and Wrekin's Local Safeguarding Children's Board has spent nearly two years working with an author to pen the serious case review into "Child B", otherwise known as Oliver Sargent.

The report was being released this afternoon at Addenbrooke House, headquarters of Telford & Wrekin Council, following talks with staff at the hospital, officers at West Mercia Police, ambulance staff and workers at Bromford Housing.

Oliver, of Priory Way, St Georges, Telford, died in Birmingham Children's Hospital in July 2012.

Paul Thomas, the boy's father, was found guilty of manslaughter and jailed for 10 years.
