Watch: David Cameron urges authorities to learn from mistakes over murderer Jamie Reynolds

Prime Minister David Cameron this afternoon urged police and other agencies who made serious failings that contributed to the murder of Shropshire teenager Georgia Williams to ensure those "mistakes" never happen again.

He was responding to a question raised by Telford MP Lucy Allan during Prime Minister's Questions in the Commons.

It was the same day a serious case review into how those involved in dealing with Reynolds and monitoring his behaviour failed to act and a case file on him was closed in 2010, just three years before he strangled 17-year-old Georgia at his Telford home.

Ms Allan said: "The brutal murder of Telford teenager Georgia Williams led to a serious case review that was published today.

"There was a catalogue of failings made by numerous agencies including the police and social services.

"We can see from the report that her horrific death could have been prevented.

'Georgia's horrific death need hot have happened. Will the Prime Minister join me in offering heartfelt condolences to Lynnette and Steve Williams and in asking all those agencies involved, to ensure they learn from this tragic case?'

Mr Cameron said: "I send my condolences to the Williams family for their appalling loss and the tragedy they have studied.

"The police and other agencies need to study this report and learn the lessons so these mistakes are not made again in future."
