Doctors left swab in patient after surgery

A swab was left inside a patient who underwent surgery at one of Shropshire’s two main hospitals, it was today revealed.
And in another case, a patient underwent an examination procedure on the wrong part of their body. The two incidents, which took place in December and are described as ‘never events’, have been reported to the board of Shrewsbury and Telford Hospital NHS Trust.
Never events are defined, as the name suggests, as incidents which should never occur.
The trust declined to give further details on the grounds of patient confidentiality.
But a spokesman today said: “One involved a swab being retained after a procedure and the other an incident of wrong site investigation. In both of these cases the situation was fully discussed with the patient and a full apology given on behalf of the trust.
“We have looked into both of these incidents and taken action that aims to prevent them from happening again. We are unable to provide any further information about these events due to patient confidentiality.”
Although rare, these were not the first ‘never events’ to be reported in recent months. Last November it was revealed that a piece of wire was left inside a patient’s chest after an operation to drain fluid at the Royal Shrewsbury Hospital.
The hospital said a doctor lost his grip on the wire, which was removed without the need for surgery.
A second incident reported in November involved a patient at Oswestry’s Orthopaedic Hospital being fitted with the wrong part during a knee replacement operation, partly because staff could not read a label properly.
The Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust said the incident was reported to the manufacturer and the labelling had now been changed. The hospital also took steps to ensure such incidents did not happen again.
