Inquest finds ‘systematic failure’ in PRH accidental death

A jury at the inquest of a Shropshire pensioner who died after falling out of bed at Telford’s Princess Royal Hospital today said his death was accidental but was caused by “individual” and “systematic failure” at the site.

Francis Steele, 89, of Ditton Priors, near Bridgnorth, died at the hospital in December 2007.

The jury of seven woman and three men today returned a verdict of accidental death at the end of a three day inquest.

They said the death was caused, or contributed to, by individual failure caused by systematic failure, which was a result of chronic staff shortages and a lack of training.
